Turn off Freeze Arrows

While at the Song Selection screen, press Left, Down, Right.

Turn on Boost

While at the Song Selection screen, press Left, Right, Down, Up.

Unlock Hidden, Stealth, and Sudden Modes

While at the Song Selection screen, press Left, Up, Right.  Each time the code is entered, it will rotate through the above modes (not in that order).
